Please submit this form for chemical waste only. List each waste container separately. Multiple containers of the same waste stream can be entered once, and then you can select actions, duplicate container. For step-by-step instructions, Tutorial here. Waste containers that are not secured with a proper cap, leaking, or properly labeled are unacceptable for pickup by EHS.
