Please submit this form for radioactive wastes only (liquid, solid, sealed sources, mixed waste, etc.). List each waste container separately.
For the "Container Contents" Section, list the isotope, ~ activity in mCi (waste container log will have the specific activity for the container), and any other constituents (e.g. eco-safe scintillation fluid).
For other wastes (chemical, biological, etc.), see the UTEP EH&S Waste webpage for their respective pickup requests. If you have mixed waste, contact EH&S for guidance on which form to complete.
Failure to complete the form with ALL information filled out in its entirety, may delay the pick-up of your waste request.
