Spill Kit Pick Up Request

Please submit this form for Spill Kits only. The container type refers to the spill type, whether it is biological, chemical, or radioactive. Waste containers that are not secured with a proper cap, leaking, or properly labeled are unacceptable for pickup by EHS.
